Certificate from the military unit confirming captivity (in the form established by the Resolution of the Cabinet of Ministers of Ukraine No. 55 dated 2023) or a document confirming the person's participation as a member of a volunteer unit (formation)

A certificate can be obtained from the Ministry of Defense, another central executive body that manages military formations, a law enforcement agency or a special-purpose state body with law enforcement functions, the commander of a unit of the Armed Forces, another military formation formed in accordance with the law or a body that is part of the security forces and defense forces of Ukraine.
The National Information Bureau can also apply for a certificate as a body that keeps records of prisoners of war.
To do this, you need to fill out an application according to the approved form and send it together with the attachments to the e-mail fd@uncp.gov.ua
